FLYING FISH VOLLEYBALL LLC is invested in not only providing a healthy outlet for physical activity, but also taking care of the mental health of our youth. In seeking partnerships compatible with this outlook, we have partnered with AAU, who in turn is a significant sponsor of PCA (the Positive Coaching Alliance). PCA promotes creating a positive experience for all athletes. FLYING FISH VOLLEYBALL LLC always has both health and safety in mind in running our program. Although safety is paramount, unfortunately there is risk involved when participating in sports, including the possibility of physical injury. Please take some time to understand the risks and concerns outlined below:
I am aware that my child would like to participate in this sport program (volleyball). I understand that participation in this sport could result in an injury inclusive of, but not limited to: fractures (i.e. arms, legs, skull, spine), contusions, concussions, lacerations, dislocations, sprains and strains. Furthermore, I am aware and understand that an accident due to this participation, may also result in permanent disability and/or death.
Due to the strenuous nature of volleyball, the athlete participating, as well as their parents, are advised to consult their physician concerning the athlete's fitness and medical health status to participate. To the best of my knowledge, there are no physical or other conditions which will interfere with my child's participation.
- Players who require the use of inhalers MUST have always it with them, even if they do not expect to need it.
- Players who are ill or feel they may be “coming down with something” and possibly contagious should not attend team activities.
I have read and understand the above risks, but as indicated by my signature on this form, I would like my child, named on this registration form, to participate in this sports program. I also consent to the emergency medical treatment for my child on my behalf in case of a medical emergency.
FLYING FISH VOLLEYBALL LLC and any athletic facilities where clinics, tryouts, training, or games will be played will assume no liability for injury or damages arising from the results of the registrant’s participation unless due to the willful misconduct or gross negligence on the part of the FLYING FISH VOLLEYBALL LLC, its affiliates, or agents.
You agree to indemnify and hold harmless FLYING FISH VOLLEYBALL LLC, and its officers, staff, and volunteers, from any demands, loss, liability, claims or expenses (including attorneys’ fees), made against them by any third party due to, or arising out of, or in connection with any of the services offered by the program on the website, or your use of the website https://www.flyingfishvolleyball.com/
FLYING FISH VOLLEYBALL LLC will follow all guidelines recommended and enforced by Federal, State, and Local Governments and Health agencies.
FLYING FISH VOLLEYBALL LLC team will have a first aid med kits available for each session. Medical release forms are stored electronically for each player.

What You Need to Know
Beginning May 19, New York State will adopt the CDC’s “Interim Public Health Recommendations for Fully Vaccinated People” for most business and public settings.
Effective May 19, most business capacities—which are currently based upon percentage of maximum occupancy—will be removed. Businesses will only be limited by the space available for patrons or parties of patrons to maintain the required social distance of 6 feet. The COVID-19 Vaccine Tracker Dashboard updates New Yorkers on the distribution of the vaccine including the number of doses received by the state, a breakdown of first or second doses, and the number of persons vaccinated with the first and second doses in each region.
In accordance with the CDC’s new guidance on mask use, fully vaccinated people no longer need to wear masks outdoors, except in certain crowded settings and venues. Find detailed guidance on mask use.
As of April 10, asymptomatic travelers will no longer be required to quarantine after entering New York.
The Open Enrollment Period for uninsured New Yorkers has been extended through December 31, 2021. New Yorkers can apply for coverage through NY State of Health or directly through insurers.
If you have been on quarantine due to contact with someone known to have COVID-19:
a negative test does NOT release you from quarantine.
It is essential to wait 10 days full days to make sure that no infection appears, even if you have no symptoms, before being released from quarantine.
Quarantine should last 10 days from the last exposure to a person with known COVID-19.
In addition, you should continue to monitor yourself for any signs or symptoms of COVID-19 for up to 14 days after that last exposure.

Program safety is a main concern. On that note, we are asking for volunteer Parent Helpers (PH) to be culture keepers for the safety of all participants in the program. PH duties will include:
- Keeping an eye out for safety hazards and “keeping the herd in the pen”
- Escorting groups to park restroom facilities and back (again, safety).
- Timekeeping to help us stay on schedule.
Parent Helpers will be background checked, and hence will need an AAU [Non-Athlete Membership] before assuming any duties (FLYING FISH VOLLEYBALL LLC will cover the AAU membership cost).
If interested joining the volunteer Parent Helpers, check the box (earlier in the registration form), and send an email to members@flyingfishvolleyball.com. Please include in the Subject: “Volunteer Parent Helper Your_Name | Player_Name”. If approved as a Parent Helper, you’ll be asked to obtain and AAU membership card for yourself and assign yourself to the club on the AAU website. We’re looking to have 2-3 PHs per Session that can alternate being the “Duty Parent” for the session.
